the provision of health services to individuals, families, and/or their communities by at least two health providers who work collaboratively with patients and their caregivers—to the extent preferred by each patient— to accomplish shared goals within and across settings to achieve coordinated, high-quality care.
Implementing Team-Based Care
The Community Guide did a systematic review of all available studies between July 2003 - May 2012 and concluded that using team-based care is an effective health systems-level, organizational intervention to improve blood pressure control. Team-based care incorporates a multidisciplinary team to improve the quality of hypertension care for patients.
